At Landmark, our interdisciplinary teams collaboratively manage our complex patient panels. These teams are led by Physicians, Nurse Practitioners, and Physician Assistants, with supporting care provided by RN Nurse Care Managers, Social Workers, Pharmacists, Behavioral Health, and other employed team members

The DELTA team Develops Exemplary Leadership, Teamwork and Accountability and serves a critical role from team member onboarding and market implementation to ongoing education and standardization. The DELTA team member serves as a “leader-coach-player” for new Landmark market employees to provide on the job training, as well as takes a role in supporting mature markets.

The DELTA Healthcare Ambassador Specialist is a pivotal member of the DELTA Interdisciplinary Team. This role actively engages with patients both in the home and by telephone. The incumbent will use compassion and empathy to earn patients’ and caregivers’ trust, persistence to support and advocate for them, and excellent communication skills to build impactful relationships. They will be subject matter experts to provide support with implementations, mature markets and providing on the job training as needed. This role reports to the DELTA Director, Health Services and may require up to 50% travel as a virtual hybrid role.

This position is full - time, Monday - Friday. Employees are required to work our normal business hours of 8:00am - 5:00pm including the flexibility to work occasional overtime and weekends based on business need and with approval.

We offer 6 weeks of training.

Primary Responsibilities:

  • Partner with clinical care management team to manage complex members in the community
  • Conduct assessments, face to face or telephonically, to assess member medical, behavioral, functional, cognitive, and psychosocial needs, i.e., health risk assessment
  • Research, locate and make referrals to community resources to address member specific care or socioeconomic needs. Follow-up to confirm services are received
  • Proactively engage the member to manage their care, provider education and coaching to support self-management of needs and health promotion as outlined in care plan
  • Assist in quality measure documentation, support, and follow-up
  • Assist in telemedicine facilitation and set-up
  • Partner with Landmark interdisciplinary team (providers, nurses, social workers, etc.) to drive improvement in patient outcomes and satisfaction
  • Interviewing and on the job training for new Healthcare Ambassadors in both implementation and mature markets.
  • Perform chart audits
  • Establish consistency and standardization of processes and promote best practices

Required Qualifications:

  • High School Diploma / GED (or higher)
  • 2+ years of experience working in healthcare setting and/or environment
  • 1+ years of experience with MS Office, including Word, Excel and Outlook
  • Access to reliable transportation that will enable you to travel to client and / or patient sites within a designated area and the ability to travel within assigned territory to meet with members
  • Ability to travel up to 50% of the time
  • Ability to work an 8-hour shift Monday-Friday from 8:00am to 5:00pm
  • Reside in Eastern, Central or Pacific timezone

Preferred Qualifications:

  • Bachelor’s Degree (or higher) in Social Work OR related subject
  • 2+ years of experience working within the local Community, with demonstrated knowledge of culture and values within the Community and familiarity with the resources available within the Community
  • CNA, CMA or experience as a Medication aide, Health Coach, Health Navigator or Healthcare Technician or role similar
  • Experience working in team-based care
  • Ability to manage patient caseload using data and reports
  • Familiarity with electronic medical record documentation

Soft Skills:

  • Exceptional interpersonal skills and superb written and oral communication skills

Connecticut or Nevada Residents Only: The salary range for Connecticut / Nevada residents is $20.00 - $35.53.
