As a Telephonic Community Health Worker (CHW), you will act in a liaison role to the high-risk OB Case Management Team to ensure appropriate resources and programs are given to members identified as pregnant. CHW’s work in a team-based structure and spend the majority of their time assisting the HFS team with member’s who are identified as pregnant, their infant through the first year of life and women of reproductive age. This role is critical to the HFS team as it serves as the administrative liaison to the pregnant population at the health plan.

Training will be conducted virtually from your home. This position is full-time (40 hours/week) Monday - Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 8:00am – 5:00pm. It may be necessary, given the business need, to work occasional overtime or weekends.

Primary Responsibilities:

  • Engage members over the phone to assist with closing gaps in care, linking to necessary services and providing education about their health as needed
  • Review available member service records and relevant documentation (e.g., utilization history, functional level, stratification information, current plan of care)
  • Data entry into community care
  • Reviewing member eligibility with HUB invoicing
  • HUB referrals and follow up
  • Entering and uploading PRAF assessments into community care
  • Identify member service needs related to identified health concerns and referral to appropriate programs and resources
  • Identify urgent member situations and escalate to next level when necessary
  • Review plan benefits and identify appropriate programs and services based on heath needs and benefits
  • Integrate health care and service needs into a plan or recommendation for member care and service
  • Work collaboratively with the interdisciplinary care team to ensure an integrated team approach
  • Identify relevant community resources available based on member needs
  • Refer members to appropriate programs and services
  • Advocate for individuals and communities within the health and social service systems

Required Qualifications:

  • High School Diploma / GED (or higher)
  • Knowledge of culture and values of community
  • Familiarity with the resources available in the community

Telecommuting Requirements:

  • Required to have a dedicated work area established that is separated from other living areas and provides information privacy
  • Ability to keep all company sensitive documents secure (if applicable)
  • Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service

Preferred Qualifications:

  • Bachelor’s Degree (or higher) in a health-related field
  • Licensed Practical Nurse (LPN), Certified Nursing Assistant/Home Health Aide, Certified Medical Assistant
  • CHW Accreditation
  • Previous care management experience
  • Experience working in Managed Care
  • Telephonic customer service experience
  • Previous employment in a medical office or other provider environment
  • Knowledge and/or experience with behavioral health or substance use disorders
  • Knowledge of Medicaid and/or Medicare population

Soft Skills:

  • Ability to multi-task and prioritize tasks to meet all deadlines
  • Ability to work well under pressure in a fast-paced environment
  • Excellent verbal and written communication skills; ability to speak clearly and concisely, conveying information in a manner that others can understand, as well as ability to understand and interpret information from others

Colorado, Connecticut or Nevada Residents Only: The hourly range for Colorado residents is $18.17 to $32.26. The hourly range for Connecticut / Nevada residents is $20.00 to $35.53.
